Please select the Club Sports team that you are interested in below and fill out the form to the best of your ability.  When you hit submit, your information will go to the Commissioner of Club Sports and Arts, as well as the coach or program director. We also encourage you to set up a campus visit through the Admissions office and let them know that you are interested in meeting with the representative of the Activity that interests you.
